Tactical breathing as a military protocol

U.S. military tactical breathing, often called combat breathing, was formalized by Lt. Col. Dave Grossman in his 2004 book On Combat. The pattern uses four equal counts: inhale for four seconds, hold for four, exhale for four, hold for four. It was designed to lower heart rate and steady fine motor control during acute stress, and it was taught for that purpose well before wellness programs picked it up.

The practice works through the autonomic nervous system. The exhale and the pause after it extend the expiratory side of the cycle, raising parasympathetic tone; heart rate can fall within a few rounds. Many veterans find a counted square easier to follow than open meditation instructions. Drill-based training under load pairs naturally with fixed parameters, and the four phases make the exercise easy to track.

That same structure can feel too sharp for daily regulation. Holding the breath after an exhale may be uncomfortable when chest tightness is already present. A paced pattern with no holds usually gives that group a gentler entry point.

Coherent breathing near six breaths per minute

Slow the respiratory rate to about six breaths per minute and the cardiovascular system changes in measurable ways. This cadence lines up with maximal heart rate variability, the beat-to-beat variation associated with healthy autonomic flexibility. The method is commonly called coherent breathing or resonance frequency breathing. A breath cycle close to ten seconds, about five seconds in and five seconds out, puts most adults near their resonance point.

No holds are used. Smooth timing carries the technique. Heart rate rises slightly on the inhale and falls on the exhale, and at six breaths per minute those movements synchronize with blood pressure rhythms. Research summarized by the National Center for Complementary and Integrative Health has linked slow paced breathing with reductions in self-reported anxiety, though effect sizes vary widely and many trials are small.

For veterans with hyperarousal, removing breath retention changes the physical feel of the practice. Retention can trigger air hunger, the sensation that often travels with panic, while coherent breathing avoids that trigger entirely. Free metronome apps and open-source pacing tracks distributed through some VA Whole Health materials allow the cadence to be set before the session starts. Once the timing is external, attention can stay with the physical sensation of breathing instead of constant counting.

The original protocol used a 45-minute scan. Shorter versions of ten to fifteen minutes are now common and fit more easily into daily use.

A typical scan may pass from feet to legs, hips, abdomen, chest, hands, arms, shoulders, neck, and face. Each region supplies the next place for attention, so the practice removes the extra task of deciding where to focus after every breath or sensation.

Chronic pain, a frequent comorbidity with service-connected injury, adds a complication. Directing attention toward a painful region can intensify perceived pain before it settles. Instructors trained through Mindfulness-Based Stress Reduction address this by teaching a separation between sensation and the reaction around it. Bracing, dread, and the wish for the sensation to stop can carry more distress than the raw sensation itself.

The scan also brings up physical information that often goes unnoticed. Many people carry tension in the jaw, shoulders, or muscles around the eyes through much of the day. Once that tension becomes clear, release becomes possible. Lying down for a scan before sleep can also serve as a wind-down routine for people whose service left them with disrupted sleep architecture.

The evidence base for Mindfulness-Based Stress Reduction in veteran populations is growing and uneven. The VA has run mindfulness groups at multiple medical centers, and outcome data point to reductions in symptom severity for some participants. Dropout rates in these programs run high, and people who leave are not always tracked.

Expectancy effects and measurable physiology

A person’s expectations influence the benefits they report from a practice, a pattern well documented across meditation research. Studies comparing an active mindfulness intervention with a credible control, one participants believe might help, tend to show smaller effects than studies comparing mindfulness with a waitlist. Waitlist comparisons inflate the apparent benefit because placebo response gets counted alongside any specific effect of the practice.

Paced breathing still produces an observable physiological shift. Heart rate variability changes appear on objective monitors regardless of belief in the method. That separates a sturdy claim from an overextended one: measurable autonomic change holds up, while the promise of curing post-traumatic symptoms through any single technique does not.

Slow breathing can earn a place in practice time because its mechanism does not depend on enthusiasm during a given session. It acts on autonomic wiring even when the session feels repetitive or ordinary.

A worked example of structuring a week

Use a fixed schedule. Six minutes of coherent breathing in the morning, at six breaths per minute, comes to 36 breath cycles. That is enough to register an autonomic shift without consuming much time, and the fixed count gives the session a clear endpoint.

Tactical breathing fits acute moments: before a difficult phone call, in a crowded space that triggers vigilance, or in the minutes after waking from a disturbed night. It is deployed during episodes of acute stress, matching the purpose for which the protocol was built.

A ten-minute body scan three times across the week, done lying down before sleep, addresses the sleep disruption that morning breathing may leave untouched. The scan has a different job from paced breathing: it gives attention a route through body sensation while the nervous system moves toward rest.

Together, the morning breathing and three scans add up to roughly 48 minutes of formal practice across seven days. The number is deliberately modest. Programs that prescribe an hour daily lose many participants within weeks, a pattern reflected in VA mindfulness group dropout data. A small practice that survives beats an ambitious schedule abandoned by the third week.

Morning breathing gives the day an early parasympathetic baseline, and the pre-sleep scan works at the handoff into rest. The schedule leaves the long middle stretch exposed, the hours when vigilance can build with no quiet interval set aside. That middle stretch is where a formal routine, even a modest one, meets the least quiet part of the day.
